CMT Flameworthy Awards 2004 (April 21, 2004). Shania Twain won the 2004 CMT Flame Worthy Female Video of the Year for "Forever and For Always." During the Red Carpet part that aired before the show, Luke Lewis of Mercury Records Nashville gave Shania the Triple Diamond Award Plaque to celebrate the first female artist ever to receive three diamond awards in a row  from the RIAA. Shania also got to introduce Dolly Parton's perforrmance during the Flame Worthy show. Shania was interviewed by Katie Cook for the show "CMT Insider" during the show.
